Please send or deliver "FOUR COPIES" of the following items to Dr. Xin Liu, 23-263 CHS, Campus Code 173517. Your application should arrive by 5PM on March 15th or October 15th. If the deadline falls on a weekend or holiday, the deadline will be 5PM of the next UCLA business day.

1. A copy of the submitted meeting abstract

2. A copy of the meeting announcement (with the meeting name, date, location, and topic)

3. The following short form signed by the graduate student
